Is Tofacitinib(Xelanz) suitable for me?

 Tofacitinib can be prescribed by a consultant rheumatologist for adults with rheumatoid arthritis or psoriatic arthritis. It can be used alone or with methotrexate.

Tofacitinib won’t be started if:

  • your arthritis isn’t active
  • you haven’t tried other treatments appropriate for your condition first.

Your doctor may decide not to prescribe this treatment if you’re pregnant, planning to try for a baby, or breastfeeding, or if you are over 65. They may also not prescribe it if you have or have had:

  • an active infection
  • repeated or serious previous infections
  • shingles
  • disease of the lungs, liver or kidneys
  • heart problems, high blood pressure, high cholesterol or blood clots (deep vein thrombosis or pulmonary embolism)
  • cancer.

Tofacitinib contains lactose, so you should tell your rheumatology team if you’re lactose intolerant.

You’ll have blood tests before treatment starts to assess your condition and whether the drug is suitable for you.

Your rheumatology team will also monitor you through blood tests once you start treatment. If your red or white blood cell count gets too low, your treatment may be stopped until it improves.

Your doctor will need to check if you’ve previously been exposed to tuberculosis (TB). Even if you don’t have symptoms, the bacteria that cause TB may still be present in the body and you may need a course of treatment for this before starting tofacitinib.

If you’ve previously had hepatitis you may need regular checks for this as Tofacitinib may increase the risk of the hepatitis coming back.

You may have to be monitored for other conditions as well, for example if you have liver problems, high cholesterol, or if you are at risk of heart or lung conditions. Tofacitinib raises cholesterol levels in some people.

What is tofacitinib (Xeljanz) and how is it used?

Tofacitinibalso known as Xeljanz, is a type of drug known as a Janus kinase (JAK) inhibitor. It works by blocking the action of Janus kinase enzymes, which are involved in the inflammation that causes the symptoms of rheumatoid arthritis and psoriatic arthritis.

Tofacitinib is a long-term treatment. Most people who benefit from this treatment will notice an improvement the first 12 weeks of starting treatment

How to used Xeljanz (Tofacitinib)

Read the Medication Guide provided by your pharmacist before you start taking tofacitinib and each time you get a refill. If you are using the liquid form of this medication, also read the Instructions for Use. If you have any questions, ask your doctor or pharmacist.

  • Take this medication by mouth with or without food as directed by your doctor, usually once or twice daily.
  • If you are using the liquid form of this medication, carefully measure the dose using a special measuring device/spoon. Do not use a household spoon because you may not get the correct dose.
  • This Xeljanz(tofacitinib) tablet dosage is based on your medical condition, response to treatment, lab tests, and other medications you may be taking. Be sure to tell your doctor and pharmacist about all the products you use (including prescription drugs, nonprescription drugs, and herbal products). Children's dosage is also based on weight. Do not increase your dose or use this drug more often or for longer than prescribed. Your condition will not improve any faster, and your risk of serious side effects will increase.

Things to know before you start abatacept

Abatacept is a very effective therapy for many patients with arthritis. However, there are certain people who may not be able to take it.

Before you’re prescribed abatacept, doctors sometimes use a scoring system to work out how active your arthritis is. The system can be different depending on what condition you have, but most often you’ll be asked how well you feel on a scale of 1 to 10 and your doctor will make a note of how many of your joints are tender and swollen.

You’ll also need blood tests before treatment starts to assess your condition and whether the drug is suitable for you.

ABATACEPT TABLET

Your doctor might not prescribe abatacept if:

  • you’re pregnant
  • you’re currently taking another biologic drug
  • you have HIV
  • you have a severe heart condition
  • you’ve had cancer.

ORENCIA Tablets should be used with caution if you have infections that repeatedly come and go.

Your doctor will need to check if you’ve previously been exposed to tuberculosis (TB). If the tests are positive you may need to start a course of treatment for TB before starting abatacept.

If you’ve had hepatitis B or C, you may need regular checks for this, as abatacept may increase the risk of the hepatitis starting up again.

What Are Side Effect And Risk Of ORENCIA (ABATACEPT) Tablets

Most people do not experience side effects. Some side effects can happen around the time of the infusion or injection. They aren’t usually serious, but if you have severe side effects or are concerned about your symptoms contact one of the healthcare professionals in charge of your care.

  • tiredness
  • headaches
  • feeling sick or vomiting
  • diarrhoea.

In very rare cases people can be allergic to abatacept. This could be in the form of swelling or a rash, or you may feel short of breath. This is very rare, but if you do develop these symptoms, or any other severe symptoms, during or soon after a dose of abatacept you should seek medical advice immediately.

Because abatacept affects your immune system, you may be more likely to pick up infections. The most common are mild chest and urinary infections.

Tell your doctor or rheumatology nurse straight away if you develop any symptoms such as a cough that won’t go away, unexplained weight loss or a fever. You should also tell your rheumatology team before having an infusion or injection if you have any of these symptoms. They may advise you to pause the treatment.

You should contact your doctor if you get chickenpox or shingles, or if you come into contact with someone who has chickenpox or shingles and you have never had chickenpox before. These illnesses can be worse than usual if you’re taking abatacept. You may need antiviral treatment, and your abatacept may be stopped until you’re better.

Tips to reduce your risk of infection

  • Try to avoid close contact with people you know have an infection.
  • Wash your hands regularly and carry around a small bottle of antibacterial hand gel.
  • Keep your mouth clean by brushing your teeth regularly.
  • Stop smoking if you’re a smoker.
  • Make sure your food is stored and prepared properly.
  • Try to keep your house clean and hygienic, especially the kitchen, bathrooms and toilets.

How is it taken ORENCIA (ABATACEPT) TABLETS?

 Orencia may be given:

  • through a drip into a vein, which takes 1–2 hours once a month, in hospital or at a special clinic. This is known as an intravenous infusion
  • as an injection under the skin once a week using a pre-filled syringe or pen. This is known as a subcutaneous injection. You, your partner, or another family member can learn to give these injections at home.

ORENCIA TABLETS



Speak to your rheumatology team if you’re already having abatacept by infusion and wish to switch to injections – you’ll probably start your injections when your next infusion is due.

If you forget to take your dose within three days, just take it when you remember to and then continue taking your doses according to your usual schedule. If you’re more than three days late taking a dose, ask your rheumatology team for advice on when to take your next one.

It’s a long-term treatment and may not work right away. Some patients report it took between 6–12 weeks before they started to feel better. So, it’s important to continue taking the medication as prescribed, even if you don’t feel better right away. You should also continue taking it when your symptoms improve, and you start to feel better, to help keep your condition under control.

Most patients will have regular blood tests while taking  ORENCIA (ABATACEPT) TABLETS. If you’re taking other drugs, such as methotrexate, you’ll also need to continue your blood tests for these.


Side Effect Of Descovy 200 Mg-25 Mg Tablet

 Diarrhea, headache, trouble sleeping, back pain, or change in the color of skin on your palms or soles of your feet may occur. If any of these effects last or get worse, tell your doctor or pharmacist promptly.

Remember that your doctor has prescribed this medication because he or she has judged that the benefit to you is greater than the risk of side effects. Many people using this medication do not have serious side effects.

Descovy 200 Mg-25 Mg Tablet


As your immune system gets stronger with HIV treatment, it can begin to fight off infections you already had, possibly causing disease symptoms to come back. You could also have symptoms if your immune system becomes overactive. This reaction may happen at any time (soon after starting HIV treatment or many months later). Get medical help right away if you have any serious symptoms, including: unexplained weight loss, severe tiredness, muscle aches/weakness that doesn't go away, headaches that are severe or don't go away, joint pain, numbness/tingling of the hands/feet/arms/legs, vision changes, signs of infection (such as fever, chills, swollen lymph nodes, trouble breathing, cough, non-healing skin sores), signs of an overactive thyroid (such as irritability, nervousness, heat intolerance, fast/pounding/irregular heartbeat, bulging eyes, unusual growth in the neck/thyroid known as a goiter), signs of a certain nerve problem known as Guillain-Barre syndrome (such as trouble breathing/swallowing/moving your eyes, drooping face, paralysis, trouble speaking).

Tell your doctor right away if you have any serious side effects, including: mental/mood changes (such as depression, anxiety), signs of kidney problems (such as change in the amount of urine, pink/bloody urine).

Rarely, this  Descovy 200 Mg-25 Mg Tablet can cause severe (sometimes fatal) liver and blood problems (lactic acidosis). Tell your doctor right away if you develop symptoms of liver problems (such as nausea/vomiting that doesn't stop, loss of appetite, stomach/abdominal pain, yellowing eyes/skin, dark urine) or lactic acidosis (such as deep/rapid breathing, drowsiness, nausea/vomiting, unusual weakness).

A very serious allergic reaction to this drug is rare. However, get medical help right away if you notice any symptoms of a serious allergic reaction, including: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, trouble breathing.

How To Give Precaution Descovy 200 Mg-25 Mg Tablet

Before taking this product, tell your doctor or pharmacist if you are allergic to emtricitabine or tenofovir; or if you have any other allergies. This product may contain inactive ingredients, which can cause allergic reactions or other problems. Talk to your pharmacist for more details.

Before using this medication, tell your doctor or pharmacist your medical history, especially of: pancreatitis, liver problems (hepatitis B, hepatitis C, cirrhosis), alcohol use, bone problems (such as bone disease, bone loss/osteoporosis, weak/broken bones), kidney disease.


Descovy 200 Mg-25 Mg Tablet


Before having surgery, tell your doctor or dentist about all the products you use (including prescription drugs, nonprescription drugs, and herbal products).

During pregnancy, this medication should be used only when clearly needed. Treatment can lower the risk of passing HIV infection to your baby, and this medication may be part of that treatment. Discuss the risks and benefits with your doctor.

This  Descovy (Emtricitabine / Tenofovir)  passes into breast milk. Because of the possible risk to the infant, breast-feeding while using this drug is not recommended. Also, if you have HIV infection, do not breast-feed because breast milk can transmit HIV.

Take this medication by mouth as directed by your doctor, usually once daily with or without food. The dosage is based on your kidney function, medical condition, and response to treatment. For children, the dosage is also based on weight.

When used to treat HIV infection, this product is taken with other medicines that treat HIV. If you are also taking didanosine enteric-coated tablets, take both products either on an empty stomach or with a light meal. If you are taking the didanosine buffered tablets, take both drug products on an empty stomach.

It is very important to keep taking Discovy (and other HIV medications) exactly as prescribed by your doctor. Do not skip any doses.

For the best effect, take this medication at evenly spaced times. To help you remember, take this medication at the same time every day.

Do not take more or less of this drug than prescribed or stop taking it (or other HIV medicines) even for a short time unless directed to do so by your doctor. Doing so may cause the amount of virus t

What is foscarnet And Its Warning

What is foscarnet?

Foscarnet is an antiviral medicine that prevents certain viruses from multiplying in your body.

foscavir(foscarnet) tablets is used to treat cytomegalovirus (CMV) retinitis in people with AIDS.

Foscarnet is also used to treat herpes simplex virus (HSV) in people with a weak immune system. Foscarnet is usually given for HSV after other antiviral medications have been tried without successful treatment.

Foscarnet is not a cure for CMV or HSV, and your virus may progress during or after treatment.

Foscarnet may also be used for purposes not listed in this medication guide.


Warnings

Foscarnet can harm your kidneys. This effect is increased when you also use certain other medicines, including: antivirals, chemotherapy, injected antibiotics, medicine for bowel disorders, medicine to prevent organ transplant rejection, injectable osteoporosis medication, and some pain or arthritis medicines (including aspirin, Tylenol, Advil, and Aleve).

Your kidney function and electrolytes (potassium, sodium, magnesium, phosphorus) will need to be tested often.

Injecting foscavir(foscarnet) tablets too fast can cause dangerous or unwanted side effects.
